David Nicholson, DO, LLC would like to introduce our new PATIENT PORTAL. This is a secure, web-based system that allows communication between you and our office at your convenience, 24 hours a day through the internet. The portal will allow our physicians and staff to communicate lab results, appointment reminders, and health maintenance reminders to our patients. When you have received a message via the portal, you will receive and e-mail alerting you to the message. In order to protect your personal health information in alignment with HIPAA guidelines, you can only access your portal messages by signing into the secure portal. Our patient portal will allow you to: · Complete new patient registration forms · Update demographic information · Update your medical record · Submit appointment requests · Submit prescription refill requests · View lab & test results · Submit a question to the nursing staff · Submit a question to the billing staff To access the portal, CLICK ON THE ICON BELOW and complete the requested information to submit your request for access through the portal. We will process your request within 24 hours of submission. |
New Patient Instructions
If it’s your first visit to Dr. David Nicholson, we strongly encourage you to complete the new patient forms available in the left-hand column of this page. This will allow us to streamline your registration process. If you’ve received obstetric or gynecologic care elsewhere in the past, please ask them to forward a copy of your prior medical records to our office so that we have all your medical information. On your first visit, we’ll review your medical history thoroughly with you, talk about your current health needs, and offer you treatment options if necessary. If at any time, you have questions, please ask us. Our staff is happy to assist you! We do ask that you arrive 15 minutes prior to your first visit so that we may better serve you. It is important that you bring your current insurance information, your driver’s license or picture id, and your co-payment with you to your appointment. Payment Information Fees We are dedicated to providing the highest quality health care at a reasonable cost. Therefore, in order to streamline our administrative process and keep our fees down, we ask that you pay for your office services at the time they are rendered. Payment may be made by cash, check, money order, Discover, Mastercard or Visa. Billing MASTERCARD, VISA AND DISCOVER ACCEPTED! Should you experience financial problems that result in delayed payment of your account, please contact our billing office at 937-901-7069. We will work with you to arrive at an acceptable plan which will enable you to take care of your obligation. All reasonable efforts are made to help you meet your financial obligations! Insurance Information We participate with many different insurance plans and will file for reimbursement. It is your responsibility to verify that we are a participating provider and to know the details of your health insurance plan with regards to deductibles, co-payments and percentage of coverage. Many insurance companies require a referral from your primary care physician to be seen by a gynecologist. It is your responsibility to acquire a referral prior to being seen. If you are unsure of your policy requirements, please contact your insurance company. Many insurance companies require pre-certification, or notification prior to hospital admission for inpatient or outpatient treatment.
